Parcliffe Medical Centre - Practice Nurse
Parcliffe Medical Centre is an
innovative and forward-thinking Practice led by our Partnership (5 GPs). Located in purpose-built premises within St Annes Health Centre, we serve and cover a local population of approximately 12,000 patients, in the seaside town of St Annes along the Fylde Coast; we are minutes away from the sea front, with direct links to the M55 & M6.
We are keen to recruit and
welcome applications from experienced Practice Nurses who would like to join our
friendly, professional, caring team who are passionate about providing safe and
effective care to our patient population.
We are open to hours of full time or part time, offering a competitive salary
based on experience. We are really keen to meet colleagues who have experience
in particular with Diabetes and Respiratory Management, alongside non-medical prescribing or an interest to develop into this.
The role is supported by an exsiting team of experienced nursing colleagues and led by our Clincal Lead Nurse.
Main duties of the job
Our Practice Nurse role would be
expected to undertake a range of nursing assessments and provide appropriate
care or treatment in conjunction with our clinical team and Practice Policies.
Ideally you'd be able to support
and deliver a high level of quality care and service based on the needs of the
patient population of which would include treatment, management plans,
screening, preventative care and patient education. In addition to this, you would
also be expected to support the Practice in meeting its contractual obligations
and delivery of enhanced service specifications.

- Assess, plan, develop, implement and evaluate programmes to promote health and well-being.
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Identify, and manage as appropriate, treatment plans for patients at risk of developing long-term condition
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
- Deliver opportunistic health promotion using opportunities such as new patient health checks
- Provide information and advice on prescribed or over-the-counter medication on regimes, side effects and interactions, as able to
