Job summary

Cygnet Primary Care Network is seeking to recruit a suitably qualified and GPhC-registered Clinical Pharmacist through NHS Englands Additional Roles Reimbursement Scheme (ARRS). The successful candidate will join our expanding multidisciplinary team (MDT), working on behalf of our member practices and based at Bartholomew Medical Group, the largest of our network practices.

Main duties of the job

The roles will be based at one of the Goole PCN sites - Bartholomew Medial Practice.

You will be responsible for the care management of patients with chronic diseases and/or recent hospital admission plus undertake clinical medication reviews to proactively manage people with complex polypharmacy especially those in care homes, the frail, and those with co-morbidities. You will be involved with shared decision making which will include involvement in regular care home rounds, prioritising patients for MDT review and detailed Care Plans. You will offer support to the Single Point of Access team that supports all the Care Homes in the network.

New roles are being introduced to the network as we are expanding into different areas of Healthcare.

The network provides a single point of access for the nursing, residential and learning disability homes in the area.

Job responsibilities

You should have a caring, dedicated and passionate approach to General Practice as well as delivering high quality medication reviews. You will also need an open and flexible attitude towards working with a variety of patient needs as well as Practice needs. Good verbal and written consultations as well as strong time management abilities are also needed.

Working within your clinical competencies alongside the general practice teams you will prescribe, be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy.

Key Areas of Responsibility

To proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with co-morbidities (in particular frailty, COPD, and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).

Deliver patient facing/remote clinical medical reviews, including long-term conditions clinics and ensure medicines optimisation. Make appropriate recommendations to GPs, nurses and paramedics for medicine improvement and monitoring.

Work with both the care home staff and PCN staff to improve the safety of medicines ordering and administration.

To offer support to the Single Point of Access team that supports all the Care Homes in the network.

Attend and/or refer patients to multidisciplinary care conferences.

Provide specialist expertise in the use of medicines whilst helping to address both the public and social care needs of patients at our PCNs Practice.

Provide training/development to primary healthcare teams on therapeutics and medicines optimisation.

Work with the practice team to ensure compliance with CQC standards.

Essential

  • Essential Criteria:
  • Completion of an accredited undergraduate degree in Pharmacy
  • Current registration with the General Pharmaceutical Council (GPhC)
  • Enrolment in, willingness to undertake, or completion of the 18-month Primary Care Pharmacy Education Pathway (or equivalent qualification)
  • Knowledge and experience of working in a Primary Care/Community Pharmacy setting

Desirable

  • Clinical Diploma
  • Membership Primary Care Pharmacy Association (PCPA)
  • Membership of the Royal Pharmaceutical Association
