Job summary

We are looking to attract a highly motivated, professional, caring and enthusiastic Nurse Practitioner to assist in the delivery of all aspects of Primary Care Services. Based in Lincoln, we are a high achieving, friendly and supportive training Practice.

Main duties of the job

The successful candidate will be a fully qualified Nurse Practitioner and Independent Prescriber with experience preferred in Primary care. Additional interest in the management of one or more long-term conditions is desirable.

JOB SUMMARY:

The post holder is an experienced nurse practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Essential

  • Registered General Nurse
  • (currently registered with the Nursing & Midwifery Council)
  • Recognised NP/ANP qualification at minimum of degree level or equivalent
  • Independent Nurse Prescriber

Desirable

  • Teaching / Mentoring experience and/or qualification
  • MSc appropriate qualification

Experience

Essential

  • Minimum of 5 years post registration experience in general practice or relevant area
  • Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of NP role
  • Experience of Health Promotion
  • Experience in management of long term conditions eg. Asthma, COPD, diabetes, CHD
  • Experience of managing minor/acute illnesses
  • Evidence of working autonomously and as part of a team
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice
  • Use of clinical computer systems

  • Interpreting and implementing local and National policy agendas for health

  • Ability to assess and manage patient risk effectively and safely
  • Good verbal and written communication skills
