Job summary

Are you passionate about making a real difference in patient care? We have an amazing opportunity for a Clinical Pharmacist to join our innovative and friendly PCN pharmacy team. We are a small but dynamic team, on the lookout for someone who's eager to support our practices and patients with safer, more effective medication management.

You will work alongside multi-disciplinary teams, with regular support from a Lead Pharmacist, clinical supervisors, and peers who are always ready to collaborate and share best practices.

You will play a key role in enhancing the quality of care we provide to our patients, working within local and national guidelines to ensure safe, effective medicine management.

Experience in General Practice is a Plus (But Not Essential) as long as you have a solid understanding of medication review processes and prescribing guidelines in primary care, we want to hear from you.

Main duties of the job

Primary care is a fast moving and adaptable environment where priorities can change to match the changing needs of our patients. The focus of the role is, to work with the practice health care teams to rationalise and manage medicines, encourage safe prescribing, and optimise medicine review processes. Whilst taking into consideration local ICB and national NHS targets and objectives.

Belper PCN is a formal association of 4 GP practices, based in and around Belper, that are working together to provide a selection of health services to our combined list of 45,000 patients.

The two keystones of our approach to healthcare are teamwork and innovation. We believe that by working together and being open to new ideas we can both improve the lives of our patients and contribute to the sustainability and growth of the primary care sector.

The PCN has a history of adopting new approaches in community care and already has an existing team of pharmacists and advanced care practitioners who have delivered real change and improvements to patient care.

  • - MPharm Degree
  • - Registration with the GPhC
  • - Minimum of 2 years post-graduate work experience

Desirable

  • - Clinical Diploma (or equivalent)
  • - Independent Prescribing qualification

  • -Excellent interpersonal skills including communication, influencing and negotiating
  • -Self-motivated, able to manage own time and work on own initiative
  • -Understand and be able to work in a multidisciplinary team
  • -Computer literate
  • - Up to date clinical knowledge and Continuing Professional Development

  • -Knowledge of local and national priorities
  • -Experience of working in Primary Care
  • -Experience of working in Community pharmacy
  • -Knowledge of GP computer systems
