Belper PCN is looking for a Community GP to work
on their established Home Visiting Service for up to 6 sessions a week and, if
wanted, time in the member practices. Your leadership and clinical prowess can
be fully applied from the start as you help develop an excellent community team
to deliver acute home visiting service, enhanced care in care homes, and
anticipatory and proactive care for older people with frailty and patients with
multifaceted health problems

Main duties of the job
We seek a General Practitioner who aspires to promote a continuous
improvement approach to leading and delivering responsive, safe, patient-centred
and effective care. The successful candidate will work alongside our EHP
Service Clinical Lead and other clinicians to develop new and responsive
clinical pathways for seamless working, challenge traditional working methods,
and proactively support individuals within the team to develop the necessary
skills, competencies and expertise to deliver the service.
We are also looking to embed the successful candidate into the clinical
teams based on each member's practice and find innovative new ways of working
to improve efficiencies for all teams. The practices in the PCN are Arthur
Medical Centre, Appletree Medical Practice, Riversdale Surgery, and Whitemoor
Medical Centre.
The post holder will lead a clinical team for
the multidisciplinary acute home visiting service. They will provide senior
clinical triage, clinical support, and mentorship where appropriate. An
interest in medical education would be beneficial

- Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits where needed
- Home visits for clinically complex cases e.g., end-of-life care, diagnostic uncertainty, complex prescribing decisions
- Senior clinical triage for the Acute Home Visiting service
- Making professional autonomous decisions in relation to presenting problems and assessing the health care needs of patients with undifferentiated and undiagnosed problems.
- Build relationships across the extended team, including, but not limited to, social care, ambulance, social prescribers, community, and mental health clinicians.
- Opportunity for involvement in service development as the team expands
- To work collaboratively with other members of the Primary Health Care Team, including participating in multi-disciplinary meetings, significant event analyses and other activities in the interest of individual and team development and improved care of patients.
- Provide holistic, comprehensive assessment and enhanced advance planning
- Maximise the best care in the patient's own home to reduce the need for hospital or care home admissions
- Provide end-of-life care in line with the patient's wishes

Promote and deliver a multi-skilled team response that includes GP Acute Home Visits, holistic assessment, care, pro-active follow up and care planning to maximise the best care in the patient's own home.

Provide medical expertise in managing older people living with frailty in the defined community.
To support ACPs and wider MDT members working in the community by providing GP clinical triage for all Acute Home Visit requests to determine the urgency and type of response needed, according to clinical need.
Support other AHPs in the team with regular debriefing sessions for patients on their caseload.
Formal and informal education is needed for the other clinicians within the team when there are difficult clinical risk decisions.
