from 14 hours a week, up to full time of 37.5 hours per week, at one or more of our 4 PCN sites. Remuneration
will be negotiable, dependent upon skills and experience.

Main duties of the job
An Advanced Clinical Practitioner
at Belper PCN will act within their professional boundaries to provide care for
patients presenting at the PCN practices, from initial history taking, clinical
assessment, diagnosis, treatment and evaluation of care. They will demonstrate
safe clinical decision-making and expert care, including assessment and
diagnostic skills.
They will work collaboratively with
the general practice teams to meet the needs of patients, supporting the
delivery of policy and procedures, and providing leadership as required.
About us
Belper PCN is a Primary Care
Network of 4 practices located a few miles north of Derby and a stones throw
from Derbyshire's beautiful Peak District. The role would involve regular work
at one or more of the practices which are based in Duffield, Belper and Horsley
Woodhouse.
The multidisciplinary team at the
PCN includes GPs, ACPs, Pharmacists, Social Prescribers, Care coordinators, General Practice Nurses,
Nurse Associates and GP Assistants. We provide high quality
primary care to our 45,000 patients from our four sites, all with ample free parking
and great patient facilities.
Communication and support are
important to us and the PCN has a great management and clinical support network
to ensure all clinical colleagues have access to all the help they need to feel
part of the wider PCN team. In addition we actively encourage career
development and there are number of PCN initiatives such as Team Up and
Enhanced Access, which provide some great opportunities for individuals to
progress their clinical practice.

Job responsibilities
To work as an autonomous
practitioner, responsible for the delivery of primary care services and also working as
part of the practice multidisciplinary team, delivering care within their scope
of practice to the entitled patient population. The Advanced Clinical
Practitioner will be responsible for a number of clinical areas such as acute on the day appointments, women's health, health promotion, and supporting practices in developing the role of the ACP.
The following are the core
responsibilities of the Advanced Clinical Practitioner. There may be on
occasion a requirement to conduct other tasks; this will be dependent upon
factors such as workload and staffing levels:
Manage patients presenting with a
range of acute and chronic medical conditions, providing expert advice to
patients
Implement and evaluate individual
specialised treatment plans for chronic disease patients
Identify, manage and support
patients at risk of developing long-term conditions, preventing adverse effects
on the patients health
Request pathology services as
necessary
Process and interpret pathology
and other test results as required
Maintain accurate clinical
records in conjunction with extant legislation
Ensure read codes are used
effectively
Prioritise health issues and
intervene appropriately and refer where necessary
Support the team in dealing with
clinical emergencies
Recognise, assess and refer
patients presenting with mental health needs
Extended and supplementary
prescriber, adhering to extant guidance
Support patients in the use of
their prescribed medicines or over the counter medicines (within own scope of
practice), reviewing annually as required
Contribute to practice targets
(QOF etc.), complying with local and regional guidance
Liaise with external services /
agencies to ensure the patient is supported appropriately (vulnerable patients
etc.)
Delegate clinical
responsibilities appropriately (ensuring safe practice and the task is within
the scope of practice of the individual)
Support the clinical team with
all safeguarding matters, in accordance with local and national policies
Understand practice and local
policies for substance abuse and addictive behaviour, referring patients
appropriately
Deliver opportunistic health
promotion where appropriate
