Job summary

Working under the supervision of the Lead Nurse and in accordance with practice guidelines and protocols, the post-holder will assist the practice clinical team in the provision and delivery of patient screening, patient education, and prescribed programmes of care.

Main duties of the job

The main clinical duties of the role will be focused on patient health checks, phlebotomy, urinalysis, ABPMs and ECGs, reviews and care plans. More details can be found in the Job Description section.

Job responsibilities

  • Clinical responsibilities
    • New patient health checks in accordance with the practice procedure
    • Height/weight monitoring
    • NHS Health checks
    • BP monitoring
    • Phlebotomy
    • Urinalysis
    • Diabetes and Asthma reviews
    • ABPM
    • ECG
    • Diabetes High risk care plans
    • Lifestyle advice to patients
    • Chaperoning duties
    • Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed
    • Ensure accurate completion of all necessary documentation associated with patient healthcare and registration
    • Ensure all consultations and treatments are accurately recorded and readcoded in the patient record and appropriate templates are used
  • Administrative responsibilities
    • Attend and participate in practice meetings as required
    • Assist with the collection and collation of data on needs related to health and well-being
    • Assist with seasonal and other special projects as requested
    • Other ad hoc tasks as required

The post-holder will participate in the training programme implemented by the Practice as part of their employment, including participation in an annual individual performance review, maintaining a record of their own professional development anddemonstrating skills and activities to others who are undertaking similar work.

  • At least two years' experience in a similar role
  • Demonstrates an understanding of the needs of the patient and role of a Healthcare Assistant
  • Ability to communicate effectively with patients and colleagues, both in person and via telephone
  • Experience of working in an environment where highly confidential information is kept and so understands the importance of maintaining confidentiality and behaves accordingly
  • Excellent attention to detail and high levels of accuracy, clinical curiosity and motivation
  • Experience of working in a busy environment and ability to multi-task and prioritise effectively
  • Excellent literacy and numeracy skills
  • Good working IT skills, including email and internet

Desirable

  • Experience of dealing with vulnerable patients
  • Strong interest in management of long-term conditions
