Job summary
We have an exciting opportunity for a clinical pharmacist to join our clinical pharmacist team. You will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy in a patient facing role. You will provide clinical leadership on medicines optimisation and quality improvement. The post holder will be supported to develop their role.
You will be employed by Linkway Medical Practice but work across other GP Practices in our Primary Care Network.
Main duties of the job
Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
You must have a minimum of 2 years experience as a pharmacist, have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
Hold or be working towards an independent prescribing qualification.
You should be able to demonstrate the ability to critically evaluate and apply research evidence based medicine into working practice.

The postholder will work as part of a multi disciplinary team in a patient facing role. The postholder will take responsibility for areas of chornic disease management within the practice and undertake clinical medication reviews to pro actively manage patients with complex polypharmacy.
The Postholder will work across the GP practice in the Primary Care Network.
