Job summary

The post holder will be responsible for the delivery of practice nursing services care to the population the practice serves. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring long-term conditions, and engaging in health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

Assess, plan, develop, implement and evaluate individual plans of care to promote health and well-being, and prevent adverse effects on health and well-being

Identify and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Identify and manage as appropriate, treatment plans for patients with a current acute health concern, such as a wound care

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care and referral on when appropriate

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care, including taking opportunities to promote health and wellbeing wherever they arise

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions were appropriate

Support and advise women requesting information relating to family planning needs and those presenting for cervical cytology consultations

Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes for both adults and children- including vaccinations for those travelling abroad

About us

We are a busy, friendly, well-established GP practice of 17,928 patients and are located in the market town of Honiton in East Devon. We have 9 GP Partners, 7 Salaried GPs, 2 Advanced Nurse Practitioners and an excellent practice nurse team that run our LTC and family planning service. We have a lead role within our PCN with additional staff that includes mental health and wellbeing coaches, social prescribing, physiotherapists, occupational therapists, and pharmacy technicians. We train GPs, nurses and medical students and we are a research practice.We are fortunate to have completed a large extension and so now have a modern purpose-built building with excellent facilities. Honiton is a lovely market town, close to East Devon coast, with excellent rail links to Exeter and London and superb local schooling.

We aim to provide a high quality, sustainable service and enable our patients to access the most appropriate healthcare through a well led, motivated and responsive primary healthcare team.
