Job summary

An exciting opportunity has arisen for a motivated, forward-thinking Practice Nurse to join our team. We are ideally looking for an experienced Practice Nurse, however, for the right candidate we will consider Nurses new to General Practice and Return to Practice candidates. You will possess a positive mindset and excellent communication skills with an ability to demonstrate a commitment to continuous professional development and contribute to a culture of continuous improvement. You will be able to work as part of a team and autonomously with equal skill.

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practice clinical team, delivering care within their scope of practice to the entitled patient population.

This is a varied role and will include Long Term Condition management, woundcare, immunisations and Cervical Screening

We are a small, friendly, ever growing practice consisting of a team of GP's, Nurse/Nurse Practitioner, Paramedic, Physiotherapist, Clinical Pharmacist and a team of admin/reception.

Job description

Job responsibilities

To undertake and oversee chronic disease management (i.e. asthma, diabetes, hypertension, coronary heart disease, COPD), incorporating progress-monitoring, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of nursing practice.

All health checks as delegated.BP monitoring. Urinalysis. Phlebotomy. Assist other clinical members when necessary. Provide lifestyle advice to patients making any necessary referrals within the Practice. All aspects of wound care including Doppler assessments and compression dressings. Record ECGs. Administer injections including travel injections and baby vaccinations as per patient group direction. Undertake high vaginal swabs as requested by a doctor. Undertake pill checks. Undertake Cervical Smear testing ( or be willing to train to do so)
