Job summary

This is an exciting brand-new role to support the Senior Community Matron t build and develop the Quality Improvement projects provided by the Frailty |Service within Healthy Corinium CIC Ltd for the South Cotswold Primary Care Network.

Main duties of the job

You will need excellent communication skills and experience of developing working relationships as well as an understanding and experience of working in the NHS with detailed knowledge of the roles of NHS organisations and of the primary care sector.

You should also have an understanding of national policy direction for primary, community and secondary care as evidenced in the NHS Long Term Plan, together with good practical and conceptual knowledge of healthcare improvement methods and practices.

You also need experience of leading and delivering projects within tight timescales, will be self-motivated, and have the ability to use initiative to decide actions and make recommendations.

Effectively deliver key projects including the South Cotswolds Virtual Community Ward MDT. This will be achieved through the effective use of information, data, knowledge and technology systems to inform and support PCN agreed initiatives and meet the needs of the local population.

To support the South Cotswold PCN in the delivery of the Quality Improvement projects,

Main duties and responsibilities

Manage the delivery of the main specifications set out in Quality Improvement plans

Implement and progress PCN QI projects such as the PCN Frailty MDT/Community Ward.

Lead on specific projects agreed by PCN and ensure they are delivered in line with project timescales.

Use recognised Quality Improvement approaches to deliver the South Cotswold PCN QI Initiatives.

Report to Senior Community Matron and PCN Clinical Director on projects as they develop and progress and to report challenges.

As required support the PCN in setting strategic goals and preparing action plans.

Liaise and work with the CCG, practices, voluntary and other organisations to implement patient services

Promote a culture which encourages teamwork, sharing of best practice and continuous improvement

Prepare and develop project plans as required

Encourage supporting team to collaborate through sharing information and intelligence across different activities.

Attend meetings to share progress and success and challenges of projects.

Raise awareness and promote all projects with all key stakeholders.
