Community Matron - South Cotswolds Frailty Service - Cirencester based
Are you an enthusiastic, motivated and dedicated
professional looking for a new and exciting challenge? We are a small, friendly
and very supportive team who are looking for an advanced practitioner to
complete our service. The South
Cotswolds Frailty Team offers a dynamic service which places each individual at
the centre of their care, delivering a person dedicated , case management approach to those living with frailty and
supporting their families and carers.
Main duties of the job
The post holder will work closely with, and be supported by,
GP colleagues and will develop close professional relationships with a wider
multidisciplinary team including Consultant Geriatricians, mental health
professionals, Social Care colleagues, Community Nurses, Physiotherapists,
Occupational Therapists and Housing Officers. You will be responsible for the line management,
supervision, training, support and leadership of a Wellbeing Coordinator within
the team. You will have the opportunity to lead on areas of service development
and deputise for, and undertake work delegated by, the Senior Community Matron
as required.This is a full time and permanent role based in Cirencester,
the hours of work are Monday Friday 8am and 6pm, with some flexibility
possible in hours. A job share may be
considered for the right applicants.

Qualifications
Essential
- Educated to Degree level or equivalent competencies in health related subjects Registered Professional Nurse
- Frailty competency broad experience working with elderly frail patients across pathway
- Highly specialist knowledge in managing patients in the community with complex medical needs, long term/ chronic disease management and palliative care
- Highly developed professional knowledge base, supported by educational development and different work environments.
- Knowledge of current and emerging health and social care economy specifically the social care agenda
- Specialist knowledge of governance and risk strategies and their application in a community setting
- What other attributes are required e.g. Innovation, Objectivity, Motivation and Enthusiasm, ConfidenceAbility to develop and maintain effective working relationships with colleagues and other partners.
- Other - include any physical skills e.g. dexterity, use of equipment, mobility as well as any physical demands, drivingKeyboard skills
- Must be proficient in clinical skills required for area of practice

- Recognised supervisory management qualification equivalent to level 3 Educated to Masters Level
