Clinical Pharmacist

Job summary

Green Meadows Surgery are looking for a new enthusiastic Clinical Pharmacist to join our practice team. We are a friendly, forward-looking Doctor training Practice with approximately 10 000 patients. Our progressive and innovative approach to the role of the Practice Pharmacist offers an exciting opportunity for the right applicant to develop their own skills, within a well-established PCN Pharmacy Team.

The ideal applicant will have excellent organisational and communication skills, relevant clinical skills and experience. Prior experience in General Practice would be desirable with independent prescribing a must.

Main duties of the job

Medicine management of acute and repeat requests and advice for patients and allied staff.

Reconciliation of discharge medication and outpatient letter requests.

Structured medication reviews via telephone and in person.

Up to date clinical knowledge and understanding of EMIS and prescribing.

Independent prescriber status

Work with local medicine management team to carry out audits and medication switches

Having involvement in ongoing training and education of allied staff and GP trainees.

Running own long term condition clinics where appropriate, and where medications need adjusting/deprescribing as required.

Managing patients with common, minor or self-limiting conditions - Minor Illness and previous GP experience desirable.

Full time or substantive part time with flexibility for right candidate.

Salary negotiable and dependent on previous experience
