An exciting opportunity for a full time Health Care Assistant who is an enthusiastic, caring and motivated individual who is committed to delivering a high quality of care to join our practice team.
We are high achieving QOF and a very forward thinking practice. We are an established training practice and a hard working team.
Main duties of the job
To provide a full range of duties, meeting patients' identified health needs, as well as those patients presenting opportunistically. Work with patients in order to support adherence to prescribed treatments and care pathways.
Support Nurse/GP run chronic disease clinics to patients (basic screening, phlebotomy) to enable ongoing management and Quality and Outcome Framework indicators QOF.
To undertake phlebotomy clinics, as well as perform ECG's, ABPM's , BP's and urinalysis.
Undertake routine and complex wound management clinics
Initiate and maintain health screening and health promotion clinics
Vaccine/Cold chain storage, monitoring and recording
Given we are a training practice to actively participate in the training of GP Registrars
