Job summary
To ensure the delivery of an effective Practice Nurse service to the practice population, delivering care appropriately according to practice policy, protocols, guidelines and procedures.
To provide guidance and support for the nursing team as Lead Nurse and liaise with GPs and Practice Manager.
Main duties of the job
- To help develop the nursing services offered by the practice and lead the nursing team in delivering these services.
- To ensure the highest standards of care are provided for patients.
- To manage patients long term conditions including but not limited to COPD, Asthma, Diabetes, Hypertension, CHD
- Provide baby / childhood vaccinations
- Women's health smears, contraceptive pills, depo injections
- Maintain clinical protocols for the nursing team
- Monitor and order vaccines and consumables for the nursing team
- Day to day management of the nursing team
- Mentor junior members of the nursing team
- Actively participate in practice and team meetings
- Lead on infection control within the Practice
About us
We are a friendly Practice with around 10000 patients and are based on sites and we are part of a large pro-active Primary Care Network of 72000 patients.
Our clinical team comprises 3 partners, 3 salaried GPs, 3 ANPS and the nursing team and we are a teaching practice with a Registrar and medical students.
We are looking for an experienced lead practice nurse to join our nursing team of 2 nurses, 2 HCAs and a phlebotomist.
The successful candidate must be experienced in primary care nursing.
The role is part time 25 - 30 hours per week tbc. We can offer some flexibility around hours but they must include a late finish one day per week.

Job Responsibilities:
Clinical
To help in the development and maintenance of Chronic Disease Registers and autonomously provide clinical nursing interventions that will support the management of both National and Local Programmes e.g. Asthma, COPD, Diabetes, Coronary Heart Disease as appropriately trained
To actively encourage, develop and provide general and specific health screening/health promotion to the practice population, with referral to the doctors as necessary.
To participate in the cervical screening programme by raising patient awareness, competently carrying out cervical sampling, and ensuring results are delivered and acted upon appropriately. To keep an audit of results for competency
To work with due regard for Health and Safety standards within the practice, including infection control procedures
To document care given and report back as appropriate
Managerial
To work closely with the practice team and all other members of the PCN team concerned with patient care
To manage the nursing team and phlebotomist with regard to annual leave and study leave ensuring adequate clinic cover.
To perform annual appraisals for nursing team and phlebotomist
To oversee staff training, ensuring all mandatory training is completed and encouraging and supporting further training to benefit the team and the surgery needs.
To plan clinics and co-ordinate with Practice Manager and other members of the Primary Care Team to meet the needs of the surgery
To continually monitor standards and evaluate care by audit, providing reports and adapting practice to facilitate improvements to outcomes
To participate in the development of protocols/guidelines and procedures
To adhere to Manor Medical Practice policies and procedures
To report any risks to the Practice Manager
To ensure staff are maintaining stock levels of treatment/clinical room items whilst being cost effective
To document clearly all consultations and treatments provided on computer
To complete all necessary paperwork and computer entries required for practice administration e.g. patient recall systems, QOF coding
To undertake any other reasonable duties within the skills of the post holder as required by the practice.
To arrange and lead Practice Nurse meetings in-house
