Job summary
The GP Link Worker plays a pivotal role in delivering long-term, person-centred, non-clinical support to individuals, their families, and carers. Working collaboratively with GP practices, voluntary organisations, and community services, the role is designed to ensure patients receive holistic, coordinated, and accessible support. The focus is on empowering individuals particularly those affected by dementia, informal carers, and veterans to manage their health and wellbeing more effectively through personalised care and connection to local resources.
Main duties of the job
The post holder will work alongside our Team Lead Katie Costa, and other clinicians within the Well Up North Primary Care Network.
The role involves providing tailored advice to individuals, delivering emotional and practical support to patients and families, building and maintaining strong links with our practices as well as the voluntary, community and neighbourhood groups. The post holder will be working as part of the Well Up North Primary Care Network (PCN). We have a combined list of over 62,000 patients across a group of 7 practices: Well Close Medical Group, Union Brae and Norham Practice, Wooler Health, Belford Medical Practice, Alnwick Medical Group, Greystoke Surgery, Gas House Lane Surgery.

- Build and maintain strong links with voluntary, community, and neighbourhood groups to map and maintain a directory of local assets, enabling timely, appropriate, and supported referrals.
- Collaborate with local partners to strengthen and sustain community-based services and groups.
- Engage in ongoing professional development, actively reviewing and enhancing the scope of the role in line with evolving community needs.
- Provide tailored advice and support to individuals with memory concerns, ensuring early engagement and access to appropriate services.
- Deliver emotional and practical support to patients and families throughout the dementia diagnosis process and as the condition progresses.
- Act as a consistent point of contact, ensuring continuity of care and fostering trusted relationships with patients and carers.
- Support carers through signposting, emotional support, advocacy, and by facilitating access to relevant services.
- Assist veterans and their families in navigating and accessing appropriate healthcare and community-based services.
