to recruit a Care Co-Ordinator and can offer flexibility of working hours of
approximately 37.5 hours per week, job share considered We welcome applications

The Care Co-ordinator for our Urgent
Care Team and Frailty at Hart Health Partnership will have a key role in
supporting the Urgent Care Team and GPs to ensure the smooth running of our
urgent care system.
Working in
our Urgent Care hub with Duty Doctor & our Practitioner team, you will be a
key link between the Urgent Care team & a range of in-house and external
colleagues, and will support the team with patient care navigation &
signposting to services that can address their further care needs.
Tasks
include but are not limited to acting as administrative support for the team,
carrying out some simple clinical tasks such as blood tests and urinalysis and
being a point of contact for vulnerable patients.
Primary
responsibilities of the role are outline on page six.
The role is
full time based at Branksomewood Road Surgery.
This post will be supervised, and line managed
by the Urgent Care Team Lead, Practice Managers and Partners.
Main duties of the job
- Contacting patients on behalf of the Urgent Care Team to convey appointments and health advice
- Assist with day to day referrals for blood tests and to other services
- Carry out some simple clinical tasks such as blood tests, urinalysisand basic Physiological observations (including blood pressure)
- Sit with patients if they are awaiting transport to hospital
- To make regular contact with frequent attenders
- Liaise with patients who are struggling due to changes in their health and social situation including patients who are elderly, have dementia, have mental health issues or a learning disability
- Support patients to prevent a crisis situation
- Signposting patients to community services
