Job summary

The successful applicant will provide leadership to a team of 3 Practice Nurses and 2 HCA's and will lead on the diagnosis and ongoing management of asthma and COPD. Evidence of training in this area will be required

You will be required to contribute to the continued professional development of the Nursing team, including the supervision of Practice Nurses, HCA's and student nurse placements

Main duties of the job

To support the practice in the efficient running and effective delivery of nursing services for the patient population of c10,000 patients

Lead on the diagnosis and ongoing management of asthma and COPD

As a senior practice nurse the expectation will be to lead by example and act as a mentor to others, and to ensure competency and safe practice within the team

Steering the team to deliver high standards in terms of CQC requirements and governance

Job Description

Job title: Lead Practice Nurse

Responsible to: Practice Manager

Location: At New Court Surgery and in patients homes

Job Summary

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. The focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. The post holder will work collaboratively with the whole general practice and attached teams to meet the needs of patients and will provide first line management of the nursing team.

Key duties and responsibilities

Clinical practice

o Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions

o Review medication for therapeutic effectiveness

o Work with patients in order to support adherence to prescribed treatments

o Implement and evaluate programmes that promote health and well-being (e.g. smoking cessation and weight management)

o Implement and participate in vaccination and immunisation programmes for both adults and children

o Advise, support and where appropriate, administer vaccinations for patients travelling abroad

o Promote and deliver evidence-based wound care and for patients presenting with aural conditions

o Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during minor operations

Delivering a quality service

o Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)

o Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures

o Prioritise, organise and manage own workload in a manner that maintains and promotes quality

o Deliver care according to the NSF and National Institute for Clinical Excellence (NICE) Guidelines and evidence-based acre

o In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local and national policy initiatives as appropriate

o Evaluate the patients response to health care provision and the effectiveness of care

o Support and participate in shared learning across the practice and wider organisation
