Job summary

We are looking for a highly motivated Advanced Nurse Practitioner to join our established nursing team to run minor illness clinics.

The role will be working alongside the On-call GP. This post would be suitable for someone newly qualified or those with more experience looking to progress their career in a supportive environment.

Excellent communication skills, a strong work ethic and team values are essential. The ideal candidate will need to be flexible with a can do attitude.

Main duties of the job

Working in accordance with the NMC, the post-holder will lead in the provision of assessment, treatment, screening, health education services and advice to patients.

The post-holder will work with reference to practice agreed protocols and guidelines to provide treatments to patients both independently and in participation with general practitioners.

The post-holder will have the required level of training and competence and, will work within patient group directions where these are available.

The role of a Advanced Nurse Practitioner within the primary care setting is to practice at an advanced level, making professionally autonomous decisions for which they hold sole responsibility.

The Advanced Nurse Practitioner will attend to people with undifferentiated and undiagnosed problems. Responsibilities will include assessment and treatment to include prescribing where appropriate, screening, and patient education.

The aim of this post is to provide patients with direct access to primary care services. The role aims to develop a balanced, skilled, clinical team, hence enhancing overall patient care.

Clinical Practice

To be first point of contact for patients requesting contact with Primary Care for specified conditions, to carry out a full health assessment of their health care needs, based on highly developed nursing knowledge and skills. To provide advice, initiate treatment, and refer to the GP on-call as appropriate.

Prescribe products to meet the needs of patients within the practice population.

To run minor illness clinics to manage early diagnosis of illness and refer as appropriate should further intervention be required.

Refer to other primary, community and secondary care members within locally developed pathways.

To work in a partnership with general practitioners, practice nurses and other professionals to develop protocols, standards, and audit tools to facilitate effective delivery of care.

Identify, assess and diagnose patients with complex health needs across the practice population.

Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs.

Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups, and individuals.

Work with others to change policies to improve health and well-being and reduce inequalities.

Lead the implementation and evaluation of evidence-based practice to improve the services offered by the practice nursing team.

Work with other members of the practice team in the implementation of specific aspects of the NSFs.

Health Promotion and Disease Prevention

To target health promotion activities according to the epidemiology/health needs of the practice population and the area indicated by the Health Improvement Programme.

To manage and audit the national and local screening programmes with the practice population.
