James Alexander Family Practice are expanding our workforce and are looking for a professional, motivated, experienced Pharmacy Technician to join our team. Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care.
The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to proactively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
Pharmacy Technicians play an important role complementing GPs, clinical pharmacists both in general practice and the community and other members of the multi-disciplinary team.
The successful applicant will join the team supported by the Clinical Pharmacist and wider practice Leadership Team to establish a successful pharmacy team.
The successful applicant will provide cover to both of our sites, at Bransholme Health Centre and Princes Medical Centre.
Main duties of the job
We are seeking a Pharmacy Technician who has completed a Pharmacy Technician course and is registered with the General Pharmaceutical Council. You will be working to establish a pharmacy team within a new practice as part of a multi-disciplinary team and therefore need a flexible approach to work.
The successful applicant will:
Act within their professional boundaries, supporting and working alongside the general practice team.
Be supported by a Clinical Pharmacist who will develop, manage, and mentor them.
Work as part of a multi-disciplinary team.
Provide primary support to general practice staff with regards to prescription and medication queries.
Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload.
Help patients to get the best from their medicines by switching medications to agreed and approved protocols, improved repeat prescribing processes in general practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce waste medicines.
Be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice team and promoting good practice in line with therapeutic developments.

James Alexander Family Practice is part of Marmot PCN which comprises of 3 GP practices based in Hull - Dr Hendow, James Alexander Family Practice and Princes Medical Centre, operating from 2 sites - Bransholme Health Centre and Princes Medical Centre and serves a population of circa 22,000 patients.
As a PCN we already have a well established Care Home team which supports our care home patients, their families and the staff working within the care home. We also deliver an Extended Access Service providing evening and weekend appointments for patients registered within our practices and for patients who are registered with another practice in Hull. We have also recently strengthened our Mental Health & Wellbeing team.
At both the Practice and PCN, our main aim is to provide excellent care to our patients and to reduce health inequalities, but in order to do this, the health and wellbeing of our staff is key. Investing in and developing our workforce ensures that we are able to continue delivering safe, effective and accessible services.

The remit of the Pharmacy Technician is
to work with the team (which includes the Clinical Pharmacist) to bring the practice and a range of stakeholders

The post holder will undertake the following duties:
Clinical
Undertake patient facing and patient supporting
roles to ensure effective medicines use, through shared decision-making
conversations with patients.
Carry out medicines optimisation tasks
including effective medicine administration (e.g. checking inhaler technique),
supporting medication reviews and medicines reconciliation. Where required,
utilise consultation skills to work in partnership with patients to ensure they
use their medicines effectively.
As determined by the practice, support medication
reviews and medicines reconciliation for new care home patients and
synchronising medicines for patient transfers between care settings, linking
with local Community Pharmacies.
Support the Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR.
Provide expertise to address both the public
health and social care needs of patients, including lifestyle advice, service
information, and help in tackling local health inequalities.
Manage Shared Care Protocols and liaise with the Clinical Pharmacist for more complex patients.
Support initiatives for antimicrobial
stewardship to reduce inappropriate antibiotic prescribing locally.

Implement efficient ordering and return
processes and reducing medication wastage.
Provide training and support on the legal,
