Job summary
We are looking for a motivated and experienced Advanced Clinical Practitioner to join our Primary Care Team.
Ideally you would be able to care for patients of all ages, cradle to grave, with an interest in children's health and safeguarding.
The post is offered as a fixed term contract initially, with a view to a more permanent arrangement if we're the right fit.
Main duties of the job
The successful candidate will work both autonomously and collaboratively within a multidisciplinary team, applying advanced clinical expertise and critical thinking skills to assess, diagnose, treat, and manage patients across the full spectrum of primary care presentations. They will demonstrate sound clinical judgement, professional accountability, and the ability to make safe and effective decisions within their scope of practice.
About us
We are a large, teaching Practice delivering high quality care to 15000 patients in Sunderland
North. The Practice is part of the Deep End Network delivering care to the most deprived in the area.
Our team consists of 3 GP Partners, Salaried GPs, ANPs,
Practice Nurses, Nurse Associates, Health Coach, HCA,
Phlebotomist, Clinical Pharmacist, Pharmacy Technician, the PCN Team and a large Admin Team. As a teaching Practice we host Foundation Doctors, GP Registrars and Pre-Registration Pharmacists.

- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients
presenting with acute illnesses for children through to the elderly
- Clinically examine and assess patient needs from a physiological and psychological perspective,
and plan clinical care accordingly
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with
complex needs
- Organise own case load and proactively visit, assess and treat complex frail patients with multiple
co morbidity who are housebound
- Liaise with other providers to ensure continuity of care to this group of patients
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a
long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and
in accordance with evidence-based practice and national and practice protocols, and within scope
of practice
- Work with patients in order to support compliance with and adherence to prescribed treatments
Provide information and advice on prescribed or over-the-counter medication on medication
regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or
emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply
principles of self-care
