Job summary

We are excited to be able to offer a role of full-time clinical pharmacist opportunity within our surgery in Ashford, Kent working for Ashford Medical Partnership. The individual will work with a highly skilled, well supported and forward-thinking clinical team. We are looking for an individual who is enthusiastic, motivated and wanting to offer the very best up to date patient care.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service.

Main duties of the job

Key duties will include:

  • Work as part of a multi-disciplinary team in a primary care setting.
  • Provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices.
  • Provide clinical input on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
  • Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

About us

The current workforce is led by 6 GP Partners, 6 Salaried GPs, 9 Allied Health Professionals, a large Nursing/HCA Team and our other highly skilled allied healthcare professionals. The clinical team is complimented by a General Manager and a large Administrative team consisting of reception, administrative and secretarial staff.

We are a friendly, proactive and forward-thinking practice and, we run our own independent PCN. As part of our recent growth, we are looking to recruit further staff members to complement our existing team.

Job responsibilities

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.

The post holder will perform face to face medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities. The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework.

The post holder will be supported to develop their role to become a non-medical prescriber, if that qualification is not already held.

Experience

Essential

  • Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
  • Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio
  • Demonstrates and understanding of, and conforms to, relevant standards of practice.
  • Working within a prior healthcare setting.
  • Understanding and interpreting data.
  • Experience of working at a senior level with stakeholders including other providers, commissioners, local authorities, patients and voluntary sector organisations.
  • Implementation of polices and procedures.

Desirable

  • Project management/coordinating and developing projects to support performance/service improvements.
  • Training administration

Qualifications

Essential

  • Completion of an undergraduate degree in Pharmacy
  • Mandatory registration with the General Pharmaceutical Council

Desirable

  • Independent prescriber
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society or Primary Care Pharmacy Association
  • May hold or be working towards an independent prescribing qualification clinical diploma
