Job summary

We are looking to recruit an enthusiastic, experienced and motivated Nurse Practitioner or Advanced Clinical Practitioner. The successful applicant will be joining an innovative and forward-thinking team, contributing to the delivery of high standards of clinical care across the catchment area served by Ashford Medical Partnerships Primary Care Network.

The post holder must hold a prescribing qualification and have the confidence to work autonomously in a General Practice environment.

The post holder will work under the direct clinical leadership of the GP Partners.

Main duties of the job

The post holder will work autonomously within their professional scope of practice, providing care for patients face to face and over the telephone.

About us

On 11th March 2019, three well established practices merged to become Ashford Medical Partnership. Willesborough Health Centre, Singleton Health Centre and St Stephens Health Centre serve a practice population of over 36,000 patients.

The practices are all located in the heart of Ashford and are approximately 2.5miles apart. The sites are within easy reach of Junctions 9 and 10 of the M20 and we are a few minutes from Ashford International train station.

Job description

Job responsibilities

Work as part of a multi-disciplinary team to deliver safe and high-quality care.

Carries out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

Delivery of care in line with local and National guidelines and evidence-based care.

Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary, secondary and other external health care providers as appropriate.

Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

Diagnose and manage acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.

Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.

  • Registered first level nurse MSc or equivalent
  • Relevant nursing/health degree
  • Extended/independent nurse prescriber

Experience

  • Demonstrable post registration experience (degree level or higher) in primary care
  • Recent primary, secondary or community experience
  • Autonomous management of minor illness, minor ailments and injuries
