Job summary

An exciting opportunity has arisen for a Young Person Health and Wellbeing Coach to join our dynamic and friendly team that is committed to delivering exemplary care for our patients.

Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the integrated care partnership.

Main duties of the job

As a Young Person Health and Wellbeing Coach you will support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what really matters to them. Helping patients and their families to develop their knowledge, confidence and skills to improve their health outcomes.

About us

Bridlington PCN is a forward-thinking network comprising of 2 practices over three sites within close proximity to each other with a total patient population of 42,000. Humber Primary Care and Drs Reddy and Nunn are both teaching practices linking into academic partners to provide ongoing support and development to the workforce.

Job responsibilities

Key Areas of Responsibility

Provide personalised support to individuals, their families and carers to support them to be active participants in their own healthcare

Empower patients to manage their own health and wellbeing through coaching and motivation techniques. Set goals that matter to them and support patients to achieve their personalised health and care plan objectives

Accept referrals and manage and prioritise a caseload in accordance with the needs of the population the PCN serves

Promote personal choice and positive risk thinking, ensuring patients understand they are accountable for their actions and decisions.

Encourage a proactive approach to the prevention of further illness, developing a greater sense of self confidence, knowledge and skills to make informed choices

Work collaboratively across the PCN, the community and other agencies to reduce health inequalities.

Discuss concerns with a relevant GP to ensure safeguarding procedures are followed appropriately.
