Job summary
We are a SystmOne forward thinking training practice looking for an experienced clinical pharmacist with proven leadership qualities who is passionate about advancing clinical pharmacy services in primary care through collaborative working. We are looking for a clinical pharmacist to join our friendly team. The role is a permanent position offering up to 30 hours per week. The successful candidate will have excellent inter-personal, organisational and teamwork skills.
Applicants are required to have excellent computer and communication skills, both face to face and telephone.
Applicants need to be smart, professional, friendly, reliable and punctual. You will need to work well under pressure, be able to multitask, pay attention to detail and have excellent customer care skills.
Flexible working is essential which may include some weekend working.
Experience of working in a GP practice is desirable.
Main duties of the job
The Practice-based Clinical Pharmacist will improve the quality and safety of care offered to patients. They will have regular contact with patients and will reduce the workload of GP prescribers in the practice. They will also support the management of patients with long-term conditions.
Specifically, the Clinical Pharmacist will develop and manage medicines optimisation services within primary care. They will work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing. They will perform face-to-face medication reviews of patients with polypharmacy; especially for those with frailty and/or with multiple co-morbidities. They will undertake face-to-face reviews of patients with specific long term conditions that fall within their competency (such as asthma reviews). They will provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship. They will contribute to practice financial stability through optimisation of medicines related targets in QOF a GMS premium specification delivery.

A large training practice in the city of Nottingham, you will be working with a team of friendly, experienced team playing an important part of a wider team, providing a first class professional service to our patients. It is very important that all staff work together as part of a team and support one another. The team consists of 7 GPs, a nursing team of 4, 4 secretaries, 9 receptionists and 1 practice manager.
Ongoing training and professional training is provided.
