Job summary

We are looking for an enthusiastic Pharmacy Technician to join a growing Practice that is willing to develop their role within Primary Care.You will join our existing pharmacy team consisting of a Clinical Pharmacist and a Pharmacy Technician. You will also work closely with our administrative team, our GP staff and more importantly, our patients to provide a continuous service.

The service to be provided will be to ensure end to end support around patients from prescribing to medication monitoring as well as offer guidance and support to the wider clinical team.

This is a unique opportunity for an engaging personality to review current patient support and continue the process of improvement with the Practice.

Main duties of the job

The main duties of the role are to provide technical support to the clinical team and other pharmacy services within general practice and Primary Care to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained.

Ensure delivery of safe, effective and efficient systems for medicines related clinical correspondence, repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Engage with patients and the wider team and provide support across multiple platforms.

Job responsibilities

Clinical

· Support the Clinical Pharmacist to conduct medication reviews and medicines reconciliation for new care home patients, and review medicines management procedures.

· Synchronise medicines for patient transfers between care settings, linking with local community pharmacies.

· Support the Clinical Pharmacist in Structure Medication Reviews (SMR)e.g. organise necessary monitoring tests prior to SMR.

· Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

· Manage shared care protocols and liaise with Clinical Pharmacist for more complex patients.

· Working with the Clinical Pharmacist, co-ordinate appropriate monitoring of high risk medication in accordance with Practices’ policy.

Technical and Administrative

· Provide hands on support and clinical/technical advice to the Clinical Teams to promote and influence high quality prescribing to minimise risk and promote safe and effective patient care.

· Work closely with the Administrative Teams to ensure coordinated and efficient processing of prescription queries, to allow GPs and Clinical Pharmacist to review the more clinically complex queries.

· Respond to medication queries from patients and staff in a professional manner, referring to the appropriate Clinician in accordance with Practice protocols.

· Monitor adherence to the Practices’ repeat prescribing policy and oversee the process of repeat prescriptions, including the further implementation of the Electronic Prescription Service and Electronic Repeat Dispensing.

· Work with Clinical Pharmacist to develop and maintain a robust recall system to ensure medication reviews are completed in a timely manner.

· Work with Clinical Pharmacist and other clinicians to develop and undertake relevant medicines audits to improve prescribing safety.

· Undertake any other additional duties appropriate to the post as requested by GPs, Clinical Pharmacist or Practice Manager.

Experience

Essential

  • Minimum 2 years
  • experience working in an NHS or similar environment.
  • Experience of working as a qualified, registered Pharmacy Technician in Primary Care, Community or Hospital Pharmacy.
  • Experience of prescribing/clinical systems such as EMIS Web, SystmOne

Desirable

  • Recent previous experience within comparable role in General Practice

Knowledge and skills

Essential

  • Understanding of Primary Care Networks, General Practice and the NHS agenda. Good clinical pharmacy knowledge, including terminology.
  • Knowledge and understanding of Pharmacy law and ethics and current legislation
  • Excellent communication skills, both verbal and written with patients and team members.
  • Good IT skills.
  • Good organisational skills.
  • Ability to work under pressure in a busy environment.
  • Ability to work both as part of a team and independently.
  • Excellent time management skills including the ability to plan, prioritise and organise workload
