Job summary

Truro PCN has an exciting opportunity for an experienced Clinical Pharmacist to join the team on a part-time basis. We are a friendly, forward-looking PCN who values the work-life balance we are able to provide allowing us to enjoy the beautiful coast and countryside of Cornwall.

Joining our clinical pharmacy team you will carry out medicines optimisation and quality improvement, providing expertise in clinical medicines reviews to reduce inappropriate poly-pharmacy and wasteful prescribing. Together with the Senior Clinical Pharmacist and your colleagues, you will have responsibility for the care management of patient with chronic diseases to proactively manage people with complex poly pharmacy, especially the elderly, people in care homes, those with multiple comorbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).

Main duties of the job

You will hold a Masters degree in pharmacy (MPharm) (or equivalent) and will be an Independent prescriber or working towards gaining the qualification. You will also have a minimum of two years post-qualification experience and have an appreciation of the nature of GPs and general practices.

Truro PCN consists of two GP Practices, both operating from Truro Health Park and together we have a dedicated team of GPs and staff caring for around 34,000 patients. We are learning practices, hosting GP trainees and Medical Students from the Peninsula School of Primary Care and the University of Exeter Medical School.

Truro PCN has a shared ambition to innovate general practice and build a sustainable model for general practice for the future. We believe in the national strategies for adding to and improving the skill-mix of our excellent primary care team and have been investing in various new roles to improve patient care including Social Prescribing and Clinical Pharmacist roles which are already effectively operating across the PCN.

Job Summary:

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in General Practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to General Practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

The post holder will be supported to develop their role to become a non- medical prescriber.

  • Excellent written and verbal communication skills
  • Is able to plan, manage, monitor advise and review general medicine optimisation issues in core areas for long term conditions
  • Able to obtain and analyse complex technical information
  • Able to work under pressure and meet deadlines

Desirable

  • Demonstrate the ability to communicate complex and sensitive information in a understandable form to a variety of audiences e.g. patients

Experience

Essential

  • Minimum 2 years post-qualification experience

  • In depth therapeutic and clinical knowledge in understanding the principles of evidence-based healthcare.
