Job summary
We are looking for a Salaried GP for 4-8 sessions, to join
our vibrant, forward-thinking team at Chiswick Medical Practice. We are a
dynamic and growing training practice serving a diverse patient population
across three sites in West London.

innovation. Our digitally enabled same-day access model, and proactive care hub
mean we are leading the way in modern general practice.
You will be part of a large multidisciplinary team, with
current specialist roles including LGBTQ+ health, menopause, diabetes, asthma,
and minor surgery. You will also benefit from structured support for your
development and wellbeing.
As a training practice we are keen to develop interests in

patient-centred environment, we would love to hear from you.
Main duties of the job
As a Salaried GP at Chiswick Medical Practice, your primary
responsibility will be to provide high-quality, patient-centred clinical care
as part of a supportive and multidisciplinary team. You will manage a broad
range of health needs in a primary care setting, including face-to-face, telephone,
and online consultations, with a strong focus on continuity and holistic care.
You will contribute to our innovative Hub model, where new
or worsening health issues are assessed promptly with same-day access and a
12-hour response time. This is supported by a robust care navigation system.
Home visits will be infrequent due to the fantastic PCN Home
Visiting Paramedic service we have available
Working collaboratively with colleagues, you will be
encouraged to participate in clinical leadership, proactive care initiatives,
and quality improvement projects to shape the future of the practice. You will
also have the opportunity to attend weekly clinical meetings to support your
continuing professional development.
About us
At CMP, we are proud of the team we have built and the
values that underpin the way we work. Our practice spans three well-equipped
sites in West London, and we are home to a stable workforce of passionate
professionals.
You will be joining a diverse multidisciplinary team,
including experienced GPs, Physician Associates, specialist nurses, HCAs,
clinical pharmacists, a proactive care team, management and operations team and
more. We work closely across the board to support each other and provide truly
joined-up care.
Our Hub model fosters close working relationships as you
will consult side-by-side with fellow GPs and clinicians in a shared,
supportive space when you work a hub shift. This encourages real-time
discussion, collaboration, and camaraderie.
We take pride in our positive, inclusive, and fun workplace
culture. From regular social events to celebrating big (and small!) wins
together, we make time to enjoy working together.
As part of our commitment to staff wellbeing we offer an
