Job summary
We are seeking a motivated and experienced Advanced Clinical Practitioner (ACP) to join our forward-thinking primary care team. This is an excellent opportunity for a clinician who is confident in independently managing a broad range of acute and chronic conditions while delivering high-quality, patient-centred care.
The successful candidate will work both autonomously and collaboratively within a multidisciplinary team, applying advanced clinical expertise and critical thinking skills to assess, diagnose, treat, and manage patients across the full spectrum of primary care presentations. They will demonstrate sound clinical judgement, professional accountability, and the ability to make safe and effective decisions within their scope of practice.
In addition to providing expert clinical care, the ACP will play an active role in clinical leadership, service development, and quality improvement initiatives. We welcome discussions around how individual interests, specialist skills, and professional aspirations can be incorporated into the role, supporting both the strategic aims of the practice and the practitioners ongoing development.

We are a large rural practice serving over 35,000 patients across the Holderness area. As a single-practice Primary Care Network, we are uniquely positioned to shape and transform the care we deliver to our local community.
We operate from seven sites across Holderness, and the successful candidate must be willing and able to travel between locations as required. Our experienced and dedicated team includes 23 GPs, a wide-ranging multidisciplinary team of healthcare professionals, and a highly valued patient services and administrative team who work collaboratively to deliver high-quality care.
