Our new Head of Projects & Performance will be responsible for developing projects, influencing the design of services, driving a culture that values quality and establishing mechanisms for monitoring practice performance. Alongside our CEO and Senior Management Team, you will also play a key leadership role.
Main duties of the job
We are looking for an experienced manager with a track record of working at senior level and of working with multidisciplinary teams. Ideally, you will have experience of general practice, possibly as a Practice Manager but the skills we are seeking could also come from an alternative career path. Above all, you must be focused on delivering outstanding care for our patients and be full of ideas to develop new projects and initiatives to continuously improve our services. You will bring a wide range of knowledge and skills to the role and be confident making decisions, leading change and effectively managing a performance.
You will need energy, drive and resilience as well as the ability to deliver under pressure. You will be proactive rather than reactive and constantly looking for ways to empower and develop our teams.

We are a large rural practice with over 35,000 patients. As a single-practice Primary Care Network, we have a wonderful opportunity to transform care for our patients. We operate from 7 locations across Holderness and this role will involve regular travel between all our sites. Our hard-working and dedicated team includes 24 GPs, an extensive multi-disciplinary team of healthcare professionals and a great patient services team.

To develop and leads projects focused on delivering outstanding patient care
To drive a high performance culture within the practice
As part of the Senior Management Team, to provide leadership and develop the organisation
As part of the Senior Management Team, take responsibility for shaping the future of Holderness Health, setting out clear plans and goals
Represent Holderness Health consistently and professionally, raising your own profile and that of the organisation within the local health community
Continuously review the performance of the practice and make bold decisions and recommendations when necessary
Lead a variety of projects as required by the business needs of the practice
Attend and contribute to Management Board meetings
Constructively challenge the Board where appropriate and suggest alternative paths
Provide cover for Senior Management Team colleagues as required during holidays and other absence
Design and implement plans to improve the efficiency or effectiveness of patient care, focusing consistently on what matters to patients
Provide strategic advice to the Management Board, CEO and Senior Management Team on all matters relating to the configuration and delivery of patient care
Collate and analyse information about the services delivered by the practice for the preparation of business proposals and bids
Policies & Procedures:
Write, review and update policies, procedures and guidelines relevant to your areas of responsibility
Ensure compliance with practice policies and procedures, effectively disseminating any changes to established policies
Clearly communicate the organisations goals and values internally and externally
Develop and maintain an effective network of contacts within the local health economy
Provide regular information and updates to staff relating to patient care, quality and performance .g. via website updates, newsletter etc.
Represent Holderness Health in external meetings and forums
Financial & Business Information:
Work with the Head of Finance & Information to set budgets for ongoing projects
Control budgets for ongoing projects and report on current and future costs and initiatives
Develop, maintain and report on practice performance and quality indicators
Collect, collate and report on a range of data relating to population health and patient care
Translate key data into a quality and safety focused patient care strategy, working with the Management Board and Senior Management Team and influencing them on the best course of action
Lead the design and redesign of how the practice delivers patient care in response to internal or external factors
Use all sources of available data to identify new ways of working, building a case for new services or pathway development
Work closely with the Network Clinical Directors to design and implement more integrated services to improve access and quality for patients
Provide direct line management to the Proactive Care Team
Support the development of projects to be implemented by the Proactive Care Team
Quality & Safety:
Maintain practice focus on quality and safety, establishing an open and learning culture
Work closely with the Head of Patient Services to ensure practice compliance with all quality and safety requirements
Ensure all teams are aware of and adhering to data quality requirements
Record and investigate serious incidents
Identify actions or service changes required as a result of serious incidents and disseminate learning
Complete service audits on a planned cycle, implement changes as required, and disseminate learning
Maintain knowledge of CQC inspection regime and work closely with the Head of Patient Services to ensure the practice performs well against all Key Lines of Enquiry
Be responsible for and committed to safeguarding and promoting the welfare of vulnerable adults, children and young people and for ensuring that they are protected from harm
Identify and act upon safeguarding concerns in relation to patients and staff
Legal, Regulatory & Contractual Requirements and Good Practice:
Ensure that Holderness Health complies with all legal, regulatory and contractual requirements in relation to the provision of patient care
Keep up to date with good practice and innovation within general practice and healthcare, applying learning within Holderness Health
Prepare reports for Senior Management Team and Management Board meetings
Complete statutory, regulatory or contractual reporting/monitoring as required
