Job summary
You will be responsible for leading our new Proactive Care Team, ensuring that we deliver excellent care by actively supporting patients to take control of their own health and well-being through both targeted and opportunistic approaches.
Managing a team of team of Social Prescribing Link Workers, Health & Wellbeing Coaches and Care Coordinators, you will ensure that proactive care is fully integrated into the practice and will use a targeted approach to addressing health inequalities and ensuring that vulnerable, deprived and difficult to reach groups have their needs met and have equitable access to local services.

Main duties of the job
You will play an active role in developing the practices proactive care strategy, determining how best your team can be utilised to provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare empowering them to manage their own health and wellbeing.
As well as managing your own caseload of patients, you will play a key developmental role in building and sustaining relationships with other agencies and the voluntary and community sector in Holderness.
You will have experience of leading a team and providing staff supervision, coaching and development. You will also have considerable experience of working in community development, adult health and social care, learning support or public health/health improvement and be able to demonstrate how you will capture and interpret data to evidence this.

We operate from 7 locations across Holderness and you must also be willing to travel between sites and/or work from home as necessary. Our hard-working and dedicated team includes 23 GPs, an extensive multi-disciplinary team of healthcare professionals and a great patient services team.
