Job summary

The role provides a great opportunity to join a committed team. You must have full Physician Associate qualifications and be registered with the Physician Associate Managed Voluntary Register (PAMVR).

Working closely with the PCN to develop the service and audit activity, you will be expected to develop links with local GPs, colleagues within the service, and in secondary care to ensure delivery of integrated, patient focused care. Working to a professional code of ethics you will be able to demonstrate flexibility in the role, and have a caring attitude towards patients. This is an exciting position and the successful applicant will need to be self-motivated and able to work on their own and as part of the MDT. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing patient care in the NHS.

Main duties of the job

You will have direct contact with patients, taking medical histories, carrying out physical examinations, updating records, making diagnoses, analysing test results, undertaking home visits and delivering treatment.

We have recently merged three practices to become Holderness Health and now serve a patient population of 36,000 patients. Our merger provides us with many new opportunities to shape services for the benefit of our local community. It is an exciting time for us as we continue to develop and build our clinical teams.

We operate from seven sites located across Holderness in Hedon, Keyingham, Patrington, Roos and Withernsea and have 19 GP partners and over 130 staff.

Job Summary:

The post holder is a Physicians Associate, who acts within their professional boundaries, supporting and working alongside a team of clinicians in general practice. In this role they will be supported by General Practitioners who will develop, manage and mentor them.

The post holder will, provide care for the presenting patient and demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice. Diagnose and treat patients who present with minor ailments and minor injuries, supported by GPs, advanced nurse practitioners and other members of the nursing team. Attend home visits. The Physicians Associate will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures. They will have the ability to prioritise and make any necessary referrals for investigations in the appropriate manner and be flexible and responsive to patient needs.

Job Responsibilities:

Clinical Practice

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Providing emergency and routine home visits.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Diagnose and manage acute conditions.

Administration and professional responsibilities:

Participate in the administrative and professional responsibilities of the practice team.

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.

Ensure the clinical computer system is kept up to date with accurate details.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.

Attend and participate in practice meetings as required.
