Main duties of the job

As a General Practice Nurse, you will:

  • Deliver comprehensive nursing care in a primary care setting, with a focus on long-term condition management, especially respiratory care (e.g., asthma, COPD).
  • Conduct patient assessments, develop care plans, and provide evidence-based interventions.
  • Support health promotion, disease prevention, and patient education initiatives.
  • Collaborate with GPs, healthcare assistants, and other professionals to ensure seamless patient care.
  • Contribute to practice development and quality improvement initiatives.

St Clements Surgery is in the centre of Winchester and provides primary care to over 16,000 patients. Our team comprises of 15 GPs, 2 Nurse practitioners, 1 Paramedic, 4 Practice nurses, 2 HCAs and a large reception and admin teams.We are also a GP training practice so have GP Registrars and Medical Students on secondment.

JOB TITLE: PRACTICE NURSE - BAND 5 (GRADE D EQUIVALENT)

REPORTS TO: LEAD NURSES/THE PARTNERS (Clinically)

PRACTICE MANAGER (Administratively)

HOURS: 37.5 hours per week over 4-5 days

Job Summary

To be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Job Responsibilities

Assist in and perform routine tasks related to patient care as directed by the lead nurse and GPs.

Such tasks may include:

  • Basic wound care / ulcer care
  • Removal of sutures
  • New patient medicals
  • Urinalysis
  • ECGs
  • Peak flow readings
  • Venepuncture
  • Blood pressure measurements
  • Routine immunisations
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician.
  • Requesting basic pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to GPs as appropriate

Other tasks including:

  • Maintaining and cleaning equipment used by the nurses and GPs.
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Undertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms.
  • Ordering of stock from the local hospital.
  • Ordering of vaccinations to maintain stock levels.
  • Participation in administrative systems in the practice
  • At all times there will be a need to maintain accurate records
  • A duty to advise lead nurse of potential problems or errors within the range of assigned tasks.
  • Attend and participate in any practice meetings when required.
  • On occasion there may be a requirement to undertake home or nursing home visits

The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

  • Using personal security systems within the workplace according to practice guidelines
  • Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
  • Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
  • Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability, and ongoing correct usage by staff
