Job summary

Fixed term maternity cover post.

An exciting and challenging opportunity has arisen to join the team based at Clare House Surgery. The post holder will work primarily with the Data Team and also provide administrative support to the wider safeguarding team members as required.

The post holder will provide a comprehensive, professional and effective administrative service to the team. Flexibility and confidentiality are key requirements of this role and a calm and friendly manner when dealing with enquiries and external organisations.

Main duties of the job

To provide general data inputting and coding support to the practice team, ensuring attention to detail and that all elements of the medical record are correctly coded.

To be responsible for undertaking a wide range of administrative duties and the provision of administrative support to the multidisciplinary team. Duties can include but are not limited to supporting with patient registration, processing of information (electronic and hard copy) in a timely manner and in accordance with current policies.

To be responsible for the Read-coding / SNOMED coding of relevant information into the electronic healthcare records of our patients on the EMIS clinical system, adhering to the practice coding policy. In addition, the post-holder will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the practice.

Job description

Job responsibilities

Scanning:

To scan all internal and external post.

To scan in-house documents such as surgical forms, registration forms, BP readings, letters from patients and any other documents.

To code scanned documents as necessary and send to appropriate department or GP.

Patient paper notes:

To receive patient notes and log into the system and file awaiting summarising.

To maintain a tidy and organised space for notes.

Emails:

To file emails to the correct department or patient record and code if necessary.

Summarising:

To summarise patient records making sure all relevant information is added.

To scan and upload documents and tidy notes.

To file notes away once summarised

To ensure all areas are clean and tidy.

Plymouth Hub:

To check daily documents and file into patient record and code if necessary.

Registration bar on EMIS:

To action tasks sent to the Data Team and complete when finished.

To assign lab reports to correct GP and file.

To check and contact patients for correct information and update where necessary.

Deductions: to locate filed paper notes, log as returned and label.

Rejections: to obtain relevant information from patient when PCSE reject patient registration to allow resubmission.

FP69s: to contact patients for up-to-date information.

To check all new registrations are complete.

FP22s to check for filed notes to be returned to PCSE or amended.

To merge duplicate records where possible.

To file incoming GP2GP registrations to patient records.

To file documents into patient records and code where necessary.

Safeguarding:

To manage sensitive information from Devon County Council and external agencies to ensure patient records are up-to-date and correct.

To liaise with Devon Council Children Services, Devon & Cornwall Police, schools and other external agencies to obtain missing information and give apologies when GPs are unable to attend meetings.

To file sensitive information into patients electronic records and add clinical codes where necessary.

To ensure vulnerable patients and families have the appropriate safeguarding codes added to their records.

To ensure sensitive information is hidden from online visibility.

To chair fortnightly safeguarding meetings with the Safeguarding Lead and take minutes and distribute as appropriate.

To add upcoming Child Protection Conference meetings to TeamNet and send reminders to GPs for GP reports to support the meeting.

To liaise with the Service Delivery Manager to allocate time for GPs to attend meetings.

To perform audits for safeguarding reviews.
