Job summary
The post holder will work directly with multidisciplinary teams to improve clinical effectiveness and person centred outcomes. The role will be based within the GP Practice providing support to the GPs and other healthcare professionals within the teams to improve the health of patients through the rational and safe use of medicines.
The post holder will contribute to the efficient working of the Practice by leading on key areas in relation to medicines and their safe and effective use. The role will include face to face and remote patient contact, as well as liaison with colleagues in community pharmacy and secondary care.

To develop and facilitate a good working relationship with all local providers of healthcare.
To liaise effectively with other health care teams concerned with the patient care as appropriate and with all other disciplines within the practice.
To plan and organise own workload, including audit and project work, and training sessions for pharmacy technician, practice team, patients, carers, etc.
To ensure that all patient information following consultations, and telephone encounters are recorded accurately.
To maintain registration as a pharmacist and comply with appropriate professional codes.
To maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.
To attend practice and other meetings of relevance as required.

About us
Peel Hall Medical Practice Medical Practice is a GMS contract-holding medical practice in Wythenshawe. The Practice has been established in the local area since the 1950s, and currently serves approximately 9600 plus patients. We have one GP Principal, seven salaried GPs, two practice nurses, one HCA and one PA. Our clinical staff also includes a dedicated Frailty Nurse and a clinical pharmacist. Peel Hall Medical Practice is a training practice, hosting GP registrars on training placements.
