Care
Coordinators are part of a GP surgerys multidisciplinary team, working
alongside GPs, social prescribing link workers (SPLWs), pharmacy staff,
nurses, surgery administrative staff etc. to provide and coordinate, an
all-encompassing approach to personalised care and promoting and embedding the
personalised care approach across the Primary Care Network
Main duties of the job

- working closely with clinical and admin team
- patient contact (Face to Face and telephone)
- Administrate Task and Duties
- patient recall for health checks, QOF, Vaccinations
- Social media overview
- running EMIS reports, creating EMIS Reports
- Batch messages via ACCURX
- Reception Desk Support
