Job summary

Main duties of the job

You will provide high-quality clinical care to patients across all age groups, managing both acute and long-term conditions in a busy general practice.

The role includes face-to-face, home visits, telephone and digital consultations, working closely with the wider team to deliver safe, effective and compassionate care.

You will play an active part in improving services, supporting clinical quality, and contributing to a positive team culture, while maintaining professional standards, continuous learning and excellent patient experience.

Job responsibilities

  • Deliver highly effective medical care to the registered population.

  • Provide services in line with the Primary Care Contract.

  • Clinically examine and assess patients physical and psychological needs and plan care accordingly, referring to other services when appropriate, using practice and local guidelines.

  • Prescribe generically and safely in line with local and national guidance.

  • Prioritise health problems and intervene appropriately in complex, urgent, or emergency situations, initiating effective emergency care when required.

  • Effectively manage patients with long-term conditions.

  • Promote health education and self-care, supporting patients to adopt healthier lifestyles.

  • Process clinical administration in a timely way, including referrals, prescriptions, results, and correspondence.

  • Review medications in line with NICE guidance, local pathways, and practice policy.

  • Maintain accurate, high-quality clinical records.

  • Share practice workload fairly and work collaboratively with the team.

  • Support anticipatory and personalised care planning.
