Job summary

To work as an autonomous practitioner, responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.

The advanced nurse practitioner will be responsible for a number of clinical areas such as minor illness, health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Job description

Job responsibilities

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Manage patients presenting with a range of minor illness, acute and chronic medical conditions

b. Implement and evaluate individual specialised treatment plans for chronic disease patients

c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Provide wound care (ulcer /Doppler etc.) to patients

f. Develop, implement and embed health promotion and well-being programmes

g. Undertake the collection of pathological specimens

h. Provide travel medicine services

i. Request pathology services as necessary

j. Process and interpret pathology and other test results as required

k. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

l. Maintain accurate clinical records in conjunction with extant legislation

m. Ensure read codes are used effectively

n. Maintain chronic disease registers

o. Be responsible for the implementation of an effective immunisation programme

p. Chaperone patients where necessary

q. Prioritise health issues and intervene appropriately

r. Support the team in dealing with clinical emergencies

s. Recognise, assess and refer patients presenting with mental health needs

t. Implement vaccination programmes for adults and children

u. Extended and supplementary prescriber, adhering to extant guidance

v. Contribute to practice targets (QOF etc.), complying with local and regional guidance

w. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

x. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

y. Support the clinical team with all safeguarding matters, in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

In addition to the primary responsibilities, the advanced nurse practitioner may be requested to:

a. Act as the clinical audit lead, effectively utilising the audit cycle

b. Supporting the IPC lead for the practice, ensuring compliance, undertaking audits as necessary

c. Monitor and ensure the safe storage, rotation and disposal of medicaments

d. Acting as a mentor to HCAs, students and newly qualified staff within the nursing team

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the practice

g. Develop areas of specialist interest, taking the lead within the practice

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA)

j. Drives the development of nursing services within the practice, liaising external agencies and professional organisations as required
