Job summary

The post holder will work as part of a multi-disciplinary team in a patient-facing role.

Proactively manage patients with co-morbidities

Provide support to general practice staff with regards to prescription and medication queries. Support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices.

Provide leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Ideally the post holder will be an independent prescriber and has experience of working in primary care and act as a specialist resource to answer medicine related queries to signpost to other services and liaise with other healthcare professionals.

Main duties of the job

Post holders responsibilities:

Hold patient facing comprehensive and complex reviews and provide medicine support

Manage medicines in discharges from hospital

Manage and assist the pharmacy team/Prescribing clerk pre-reg pharmacists

Hold long-term conditions management within general practice: e.g. Diabetes Hypertension, respiratory medicine

Provide patient telephone support

Be a centre of medicines information for practice staff

support and adhere to repeat prescribing processes

Complex Diabetes management including insulin initiation and titration

Manage MHRA and CAS Alerts

Lead in medicines management within GP practice-audits QIPP clinical governance, drug safety searches

Training, and development and supervision of newly qualified Pharmacists and pre-registration pharmacists

The successful applicant will join an enthusiastic and supportive team of 4 GP partners 2 ANPS, 3 salaried GPS and Registrars. The successful candidate will have the opportunity to participate in all aspects of patient care, including QOF proactive care and participation in our regular practice meetings and learning events.

In addition to the clinical team, the successful applicant will also be supported by a well-developed management structure and reception team to support in delivery of high-quality patient care. Johnson Medical Practice was found to be Good in all areas of our recent CQC inspection.
