Job summary

We have a vacancy for a qualified Healthcare Assistant. You will be to able to carry out Diabetes foot checks, New patient health checks, NHS health checks, blood pressure checks and flu vaccines as well as other HCA duties. Ideally you will have experience of carrying out these types of reviews or checks within a General Practice setting.

You will need to be able to use computerised notes on our clinical system EMIS Web.

Salary is dependant on experience. Hours will be up to 20hrs per week.

Main duties of the job

HCA qualifications and updated training certificates for CPR, infection control and flu vaccines.The ability to check BP, foot check and carry out reviews such as diabetes. Ability to use computers. Training will be provided for EMIS web if needed.

The successful applicant will be expected to meet the English language fluency requirements as outlined in the Immigration Act 2016.

The right candidate will be supported by an excellent clinical and administrative team. We are situated in a purpose built building which is shared with the North East London Foundation trust (NELFT). We are also a teaching practice.

The post holder will be required to carry out a wide range of duties under the direction of the GP, Practice Nurse and Practice Manager to maintain high quality service delivery to our patients.

Job Summary:Working under the direct supervision of the GP, assisted by the Practice Nurses, and working as part of the clinical team and strictly in accordance with specific Practice guidelines and protocols, the Healthcare Assistant will assist the Practice clinical team in the provision and delivery of prescribed programmes of patient care.

Duties and Responsibilities:

To provide high quality patient care and to work as a supervised member of the health care team, performing delegated nursing duties, for example:

ECG recording

Assisting with flu clinics (under strict protocols, with up-to-date training and monitoring from GP/PN)

Blood Pressure Monitoring

Chronic disease monitoring

Measuring lung function / spirometry

New patient checks

NHS Health checks

Smoking service

Diabetic foot checks

HIV testing

Weight Management

Urine testing

Chaperoning duties

Summarising patient notes

Assist in the achievement of QOF and taking ownership of particular clinical areas

Vaccine/cold chain storage, monitoring and recording

Clearing and re-stocking consulting rooms

Preparing and maintaining environments and equipment before, during and after patient care interventions

Assisting GPs during the performance of minor operations and coil fitting

Assisting in the assessment and surveillance of patients health and well-being

Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual

Helping to raise awareness of health and well-being and how it can be promoted

Assisting with the collection and collation of data on needs related to health and well-being

Health promotion displays

Data entry into the computerised clinical record database for QOF and Enhanced Services Requirements.

Referring patients as appropriate eg. National Diabetes Prevention programme, retinal screening, pulmonary rehabilitation, lifestyle guidance, social prescribing etc.

Nursing home visits for health checks and appropriate vaccines

Participate in audit to help achieve targets eg. QOF, enhanced servcies and monitor own work.

Undertake wound care (with appropriate training and supervision)
