Job summary
It is an exciting time to join primary care, the NHS Five
Year Forward View (2014) put GP surgeries at the heart of the vision for
improved healthcare. The transformation of local healthcare and the formation
of Primary Care Networks (PCNs) has provided the opportunity for pharmacy teams
to work alongside a range of health professionals to play a key role enhancing
health, improving safety, and advancing quality.
You will be enrolled onto the Centre of Pharmacy Postgraduate
Education (CPPE) primary care pharmacy education pathway or hold an equivalent
qualification. You will be supported to complete the pathway and attend the
training days over the first one and half years if you have not completed this
already. If you are not already a
prescriber you will be supported to become an independent prescriber once the
first 18 months of the CPPE pathway have been completed
Main duties of the job
The role will encompass clinical, leadership, educational and administrative elements. There will be a mix of face to face, telephone, and video consultations with patients. This is balanced with desk based and inward facing duties, providing a varied and interesting working day.
About us
Wyvern Health Partnership is a newly formed partnership of five GP practices, formed in 2018 to improve the quality of care for over 50,000 patients across the area.
The partnership incorporates Hawthorn Medical Centre, Lawn Medical Centre, Merchiston Surgery, Old Town Surgery and Priory Road Medical Centre.
Our very busy practices have fantastic teams, each led by a group of partners and a practice manager. They include training practices for GPs and nurses.
The practices came together in response to the increased demand for primary care services in Swindon and the considerable pressure this put on resources, and on staff in particular.
The partnership has 23 partners and a large team of GPs, nurse practitioners, general nurses, health care assistants and support staff, working together and sharing best practice to benefit patients.

Job responsibilities
- To handle day to day medication issues highlighted by patients, staff, or other professionals within your area of competence
- Liaise with the PCN technician and community colleagues on out of stock items
- Within your scope of competence carry out structured medication reviews (SMRs) for patients
- To educate and support patients in the use of their medicines and optimising their health
- To perform medicines reconciliation for patients on discharge, after clinic appointments and at transfers of care
- See patients new to the PCN to review their medicines and their care
- To facilitate switches of medication in line with local formulary choices or changes in practice within your area of competence
- Construct or use existing searches to identify patients on high risk medicines and ensure that monitoring and review is carried out in line with guidelines
- Develop mutual working relationships with colleagues in Wyvern Health Partnership
- Working with senior pharmacists carry out searches for people on medicines that require a shared care agreement (SCA)
- Work with colleagues in WHP in promoting consistent ways of working and introducing new ways of working to WHP
