Job summary

Respiratory Nurse Specialist

To work with our multi-disciplinary clinical workforce as part of Meon Health Practice offering long term condition management, expert care, advice, support and education to patients with respiratory disease and focussing particularly on health improvement.

You should have advanced skills in the assessment and management of patients with respiratory conditions and have excellent organisational skills.

Main duties of the job

Work independently delivering care to respiratory patients who have been diagnosed with Asthma/COPD

Provide a high level clinical assessment, care planning, expert advice, extended intervention and rigorous evaluation.

Advanced knowledge of the management of respiratory conditions

Acts as an expert practitioner providing clinical support and advice to healthcare professionals where necessary to assist them in the management of Asthma, COPD and other respiratory conditions

Be responsible for the assessment, planning, implementation and evaluation of specialist programmes of care.

Provide direct, skilled and individualised care for patients and their carers.

Act as a specialist resource for patient and their family providing care and support that is responsive to their needs in particular the effective management of their individual condition

Communicate difficult complex information to patients, families and carers.

To work collaboratively through effective communication with multi-professional teams to ensure practice is safe, efficient, effective and evidence based.

JOB SUMMARY:

To work with our multi-disciplinary clinical workforce as part of our organisation offering expert care, advice, support and education to patients with respiratory disease, focusing particularly on health improvement.

RESPONSIBLE TO: Allocated GP

ACCOUNTABLE TO: Managing Director

KEY/MAIN RESPONSIBILITIES:

Work independently delivering care to respiratory patients who have been diagnosed with Asthma/COPD

Provide a high level clinical assessment, care planning, expert advice, extended intervention and rigorous evaluation.

Advanced knowledge of the management of respiratory conditions

Acts as an expert practitioner providing clinical support and advice to healthcare professionals where necessary to assist them in the management of Asthma, COPD and other respiratory conditions

To develop a comprehensive service that is flexible and innovative meeting the needs of the client group.

To apply standard principles of infection control to all activities at all times

Contributes to feedback of practice audit data and to the development of action plans to improve practice

Skills

Excellent communication and interpersonal skills

Advanced skills in the assessment and management of patients with respiratory conditions

Ability to work and make clinical decisions as an autonomous practitioner

Ability to cope with and diffuse difficult and stressful situations.

Excellent organisational skills.

IT skills

Primary care experience is desirable

Qualifications

Relevant Nursing degree

Masters level education is desirable

COMMUNICATION

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

DELIVERING A QUALITY SERVICE

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)

Produce accurate, contemporaneous and complete records of patients consultations, consistent with legislation, policies and procedures

Deliver care according to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence based care

Assist in the formulation of the Partnerships strategy and policy

Manage and assess risk within the areas responsibility, ensuring adequate measures are in place to protect staff and patients

Undertake mandatory and statutory training

Apply infection control measures within the Partnership according to local and national guidelines

ADMINISTRATIVE

To ensure accurate notes of all consultations in the patients computer notes

To ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice

Attend and participate in nurse meetings

RESEARCH AND AUDIT

  • Co-operate and participate as required in any research or audit in the practice

LEARNING AND DEVELOPMENT

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development

  • Contributing to strategic planning for the nursing team
  • Developing nursing service (as individual and as part of team) in line with the Partnership
  • Developing, agreeing and reviewing protocols where appropriate
