    Job description

    Job responsibilities

    Clinical responsibilities

    Maintaining the highest clinical standards at all times and in accordance with accepted good medical practice, relevant National and local clinical guidelines and various clinical protocols of the practice.

    Encourage and support the GP provision of the services to promote the medical and clinical services delivery in a timely manner as per CCG Practice contract.

    Consulting with patients both face to face and over the telephone. Transferring of relevant clinical information of patients to other medical and nursing staff both within and external, regarding patients under care of those referred to other places for further treatment of follow-up.

    Recording all contact with patients into the patients records on the electronic medical records system (the system used by the practice is EMIS Web)

    Making appropriate referrals for patients via the Choose & Book system. Using the computer to generate all repeat prescriptions including RA Repeat Dispensing and prescription for Blister packs for patients ONLY for very old and disabled patients.

    Taking appropriate and necessary action with the blood tests, urine test, x-ray and smear tests etc results which are received via the lab links. Calling and informing patients of their results wherever necessary.

    Reading the mail/post which is received. Making comments and also entering them onto the system in the Consultation mode. (ie Externally)

    Summarising briefly the hospital letters and summaries which are received for patients registered at the practice and recording them on the Consultation screen of the medical records of the patients.

    Undertake home visits, and care of practice patients in

    nursing homes or residential homes. When home visits are undertaken entering them in the consultation as Home Visit on the patients medical records. The prescriptions issued by hand during a home visit should be generated as O outside or B by hand.

    You will take the necessary steps to action the Alerts which appear for the patients during your consultation.

    You will play an active role in helping the practice to achieve the maximum QOF points during the year. Undertake the review of defined patient group and update the clinical data on the computer templates.

    Undertake annual appraisal and revalidation. Contribute to the development of ideas, strategies and policies which will promote efficiency and well-being of the service delivery by attending practice education meetings, business meetings, and PHCT meetings. Keep up to date with advances in medicine and practices.

    Promote the application of standards, protocols and codes of practice as set by Royal College of General Practitioners, General Medical Council, all other RoyalColleges and Councils applicable to the staff within the practice, Department of Health, Primary Care Trusts and all other regulatory and standard setting organizations

    Assist in the delivery of the service and promotion of policies made by the practice

    Ensure patient notes are fit for purpose, information entered on EMIS and filed in accordance with appropriate protocol

    Good prescribing and medicines management.

    Be supportive of any complaints policy and procedure to ensure proper investigation and appropriate follow-up action in the event of a complaint being made against any aspect of the service provided or any personal provider of the service

    Completing disability forms, Blue Badge forms, reports for Insurance purposes, DVLA forms, etc for patients who have consulted with you. The payments for this additional work can be claimed by you.

    When doing 6/12 repeat dispensing you must check the BP, Blood tests, PFR, Medication review, & the next due date.

    Administrative Duties

    You will take part in your PDP and attend relevant training events and workshop organised by the local Providers and the PCT

    You will work with all members of the primary health care team, to assist the practice in delivery quality care to the registered patients.

    You will attend the Practice and the Primary Health Care team meetings held in the practice and contribute constructively towards them

    You must be willing to undertake additional courses or training to meet the needs of the practice population and adapt working practice to meet the changing circumstances and service developments
