Job summary
Plas Ffynnon Medical Centre plan to strengthen their clinical team in 2026 with the addition of the role of Advanced Clinical Practitioner.
Experience of working in a primary care setting is preferred but we would support an ACP without primary care experience with the provision of an identified GP supervisor, daily support from the rota supervising GP and mentorship with other appropriate clinicians.
The post is for 30 hours per week to include Monday and Friday. Please apply with a CV and covering letter.
Main duties of the job
The ACP will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients. Although part of the nursing team, the ACP will work collaboratively with the entire general practice team to meet the needs of patients, supporting the delivery of policy and procedures. To work at this level the successful candidate will hold a masters degree in advanced clinical practice.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs and plan clinical care accordingly.
Diagnose and manage acute conditions, integrating both drug, and non-drug-based treatment methods into a management plan.
Prescribe and review medication appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
About us
Plas Ffynnon Medical Centre is a welcoming and friendly team led by 5 GP partners and a manager partner. The GPs are supported by 2 salaried GPs, a very strong nursing team into which we will introduce a reliable and dedicated Advanced Clinical Practitioner. We are a training practice, currently supporting 3 registrars with their training. The GPs are also supported by an experienced Physicians Assistant. We provide care to over 9500 patients and are part of North Shropshire Primary Care Network (PCN). We are proud of our caring ethos and the friendly and supportive environment within which our team works. We work collaboratively with the other 2 practices in Oswestry and operate from purpose-built premises with parking facilities for staff. We are rated Good by the CQC.
We are an EMiS practice and are about to introduce Anima Workflow. In early 2026 we will be introducing a triage model of care.
