Practice Nurse - 3 days (25 hrs p/wk)

Job summary

As part of an expansion of the clinical team and an investment in patient services, we wish to employ an experienced and energised Practice Nurse who actively contributes to the clinical and wider practice team to provide the highest standards of care and compassionate service to our patients.

Main duties of the job

  • Provide high-quality nursing care and support to patients at both our sites
  • Deliver a full range of treatment room services including wound care, immunisations, cervical cytology, ear irrigation, and health promotion
  • Support chronic disease management, ideally with a special interest or training in either diabetes or respiratory care
  • Act as the IPC lead at Brailsford (working in collaboration with the IPC lead at Ashbourne)
  • Work collaboratively with the wider clinical team to ensure continuity and quality of care
  • Be willing to undertake further training and development to expand clinical skills

What we are looking for:

  • NMC registered Practice Nurse with experience in primary care
  • Ideally trained in respiratory or diabetes management (or willing to train)
  • Confident in delivering treatment room services
  • A team player with excellent communication and organisational skills
  • Able to work independently across both sites

We use the S1 clinical system.We offer competitive terms and conditions and are eager to support the right candidate in all opportunities to develop their skills. Contracted hours of work 3 days a week, up to 25 hours per week, to be discussed at interview.

About us

Henmore Health is a positive and dedicated provider of primary care in the Ashbourne area, the gateway to the Peak District. We comprise 2 practices located 7 miles apart Henmore Health The Surgery Ashbourne and Henmore Health Brailsford. This position will cover both practices (likely 1 day at Ashbourne, 2 days at Brailsford).

We are high performing SystmOne dispensing practices, serving a combined growing rural community of over 13,500 patients.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

To be aware of current local and international guidelines, and attend updates as required.

To act as the Infection Prevention Control (IPC) Site Lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the Local Governance Officer.

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

To provide chronic disease clinics, delivering patient care as necessary - implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing,referring patients to secondary/specialist care as required

To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF, to include wound care, ear irrigation, venepuncture, electro-cardiography, administering immunisations and vaccinations, and any other procedures such as have been agreed with the practice and for which training has been provided

Undertake health screening procedures, eg. cervical cytology, and give patients appropriate health promotion advice

To assist in the provision of care for housebound patients with chronic diseases to the same standards as those able to attend the surgery. Also, to attend to patients requiring INR monitoring either at surgery or in the home setting if patient is housebound

To prescribe medication for patients in accordance with PGDs and own scope of practice

To review patient medications to enhance compliance, and to provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages (within own scope of practice)

To request pathology services and process pathology results as required

To maintain accurate clinical records in conjunction with existing legislation, ensuring that coding of data is used effectively

To ensure the appropriate maintenance and use of chronic disease registers

To assist GPs with minor surgery where required

To support the team in dealing with clinical emergencies

To recognise, assess and refer patients presenting with mental health needs

To liaise with external services/agencies to ensure the patient is supported appropriately (referrals to wound care, vulnerable patients etc.)

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

To support the clinical team with all safeguarding matters, in accordance with local and national policies

To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

To deliver opportunistic health promotion where appropriate
