Job summary

This is an exciting opportunity to join Henmore Health. We are expanding our mental health outpatient team in the Primary, Community and Occupational setting. The MHP will operate outpatient/outreach Mental Health clinics, treating patients in the Primary, Community and Occupational setting and referring where necessary.

We will consider a range of models of working to suit the right candidate(s), be this a flexible approach or a job-share, 1-6 sessions per week. You will be based primarily at the Ashbourne Branch, Henmore Health The Surgery, Derbyshire.

Main duties of the job

The post holder will be a CPN by background with advanced Eye Movement Desensitisation Reprocessing (EMDR) skills, as well as Cognitive Behavioural Therapy (CBT) and experience in delivering adult and adolescent care.

You will:

  • Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.
  • Undertake formal mental health and social care assessments of new clients
  • Assess, care-plan, provide interventions and evaluate and record community mental health and social care delivered to clients on own caseload.
  • Provide effective care coordination to an identified caseload of complex service users, ensuring the delivery of evidence based interventions to encourage successful outcomes.
  • Encourage and support clients to collaborate in their treatment programme and recovery.
  • Act in a consultative role/advisor to partner agencies and personnel in relation to individual clients and their mental health issues.

  • Triage including telephone triage
  • Reports for use in Legal cases

Desirable

  • Knowledge of occupational health issues
  • Knowledge of Defence or Public Service occupational issues and operations

Qualifications

  • Relevant mental health nursing qualifications at degree level or equivalent.
  • High level EMDR Training
  • CBT Qualified
  • Adolescents and adult competencies

  • Human Performance training
