Job summary

This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.

Main duties of the job

The nurse will be responsible for a number of clinical areas such as care homes and nursing homes ward, MDT meetings, personalised care plans, chronic disease management, immunisations, health promotion and prevention. They will work as an autonomous practitioner and use their specific expertise and expert clinical knowledge to assess and support care homes to plan, implement, and evaluate nursing care.

The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

This organisation must ensure that the post-holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

This is a busy practice, with a list size of approximately 13,200 patients and an above average elderly population.

We have a highly skilled and experienced clinical team including GPs, Advanced Nurse Practitioners, Practice Nurses, HCA's, Nurse Associate, Practice Pharmacist and a Clinical Care Co-ordinator.

Job Responsibilities:

Clinical Duties

  • a. Consult with patients and their families to undertake assessments of patient need
  • b. To provide expert clinical case management and support for patients with complex care needs living in Nursing & Residential Homes, who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission.
  • c. As part of this role, you will undertake regular ward rounds at care homes and nursing homes.
  • d. Deal empathetically with sensitive information, to include discussing patients changing condition with patient and/or carer.
  • e. You will diagnose, prescribe, and implement a range of core interventions with an emphasis on avoiding hospital admission or an unnecessary longer length of hospitalisation.
  • f. Actively case-manage by advising and co-ordinating on preventative health care input to vulnerable care home residents.
  • g. Planning care interventions for people with multiple and complex long-term conditions, working with the multi-professional team. You will work collaboratively with Care home and Nursing home staff and the practice team in managing the health needs for the designated patient group
  • h. Able to provide the interface between hospital and primary, community and social care settings
