We are keen to recruit a Practice Nurse to join our existing highly skilled nursing team, working alongside our Advanced Nurse Practitioners, Practice Nurses and Healthcare Assistants.

Main duties of the job

The ideal candidate would have worked in primary care, with experience in sexual health, vaccinations (including children) and managing chronic diseases.

Applicants would need good interpersonal skills, and be flexible and reliable.

JOB DESCRIPTION

Job Title: Practice Nurse

Responsible to: Drs Whittle, Dr Bevz, Dr Fairhead, Dr W Yousaf, Dr F Yousaf and the Practice Manager

Clinical Duties and Responsibilities

In collaboration with the Nurse Practitioners and the Doctors, the Practice Nurse will primarily be responsible for maintaining the practices high standards of the Quality and Outcomes Framework and the provision of care for patients in the following areas:-

Chronic Disease Management:

  • Diabetes
  • Ischaemic Heart Disease/TIA/Stroke
  • Hypertension
  • Respiratory

Well Women Screening

  • Contraception
  • Depot injections for contraception
  • HRT following practice protocol
  • Cervical cytology (communicate with patient for follow up and repeat smear as appropriate)
  • Sexual Health

General Duties:

  • Travel vaccination/Immunisations and advice
  • Flu Clinics

Carry out all other screening and diagnostic tests which the Doctors deem necessary, including:-

  • Taking blood samples
  • Carrying out ECGs
  • Near patient testing

Health Checks

  • NHS Health Checks
  • Learning Disability Health Checks

Other tasks will include:

  • Maintaining GP and Nurses rooms, stocking and rotating items as required
  • Chaperoning and assisting patients who are being examined by another clinician
  • Maintain general tidiness and cleanliness of nurses rooms
  • Maintain a high standard of Infection Control
  • Ordering of Health Promotion Literature
  • Ordering of vaccinations to maintain stock levels
  • Participation in administrative systems in the Practice
  • A duty to advise Nurse Practitioners/Doctors of potential problems or errors with the range of assigned tasks
  • Attend and participate in any Practice meetings when required
  • On occasions there may be requirement to undertake home visits
  • Any other delegated duties appropriate to the post

Collaborating closely with the Practice Manager re:

  • Involvement in risk management and significant event audit
  • Targets for Quality and Outcomes Framework
  • All necessary GMS contract targets are completed satisfactorily

To maintain routine maintenance of practice equipment

  • The Practice Nurse will monitor fridge temperatures and ensure that vaccines are stored correctly.
  • Helping the whole of the Practice Team to maintain accurate data on all patient records, including entering information on to patients computer details.
  • Maintaining good communication with all members of the Practice Team, including District Nurses and all other attached to the Practice Team.

Continuing Education

  • Continue to update through relevant literature, especially in relation to NSFs and NICE recommendations.
  • Maintain awareness of clinical governance issues.
  • Participate in clinical supervision
  • Contribute to practice based guidelines, protocols and the practice development plan.
  • Annual appraisal and to undertake development identified here.
