We are keen to recruit an Advanced Nurse Practitioner to join our existing highly skilled nursing team, working alongside our Practice Nurses and HCAs.

Main duties of the job
The ideal candidate will have experience of working in Primary Care / General Practice, demonstrating capabilities in managing own caseload, ideally with experience in some aspects of paediatric care, acute/ same day presentation and aspects of chronic disease.
Independent prescribing status is preferable.
The role will involve undertaking some aspects of triage and telephone consultations as well as contributing to our Quality Outcomes Framework.
Applicants would need good interpersonal skills, and be flexible and reliable.

Job Title: Advanced Nurse Practitioner
Responsible to: Drs Whittle, Dr Bevz, Dr Fairhead, Dr W Yousaf, Dr F Yousaf and the Practice Manager
Salary: Depending on experience
Job Summary:
Diagnosing and treating patients with acute and chronic conditions, advising in respect of their continuing medical and nursing needs. Patients will have booked appointments at fifteen-minute intervals. The nurse practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. Undertake medication reviews, respond to patients queries and action out of hours summary reports.
Job Responsibilities: Clinical Duties
The Nurse Practitioner makes professionally autonomous decisions, for which she will have sole responsibility. Has the authority to admit or discharge patients from their caseload and refer them to other health care providers as appropriate.
Provide Same Day Consultations
- Triage and treat patients, making any necessary referrals to other members of the primary health care team
- To provide same day consultations, this includes taking appropriate detailed history from the patient and their families, undertake appropriate physical examination, initiate diagnostic tests and plan the necessary intervention and treatment.
- The Nurse Practitioner will receive their own laboratory results and make the appropriate action on the patients notes. The Nurse Practitioner will also supervise and advise the Practice Nurses on issues surrounding the interpreting results.
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
Provide Telephone Triage
- The Nurse Practitioner is responsible for sorting out the requests for same day consultations, this includes determining whether the callers condition can be managed through advice, whether they need to come in and see a health professional or whether they can be prescribed medication over the telephone.
- To undertake home visits where appropriate.
Provide and co-ordinate services for the management of minor injuries and ailments.
Providing assessment, screening treatment services and health education advice:
- Diabetes
- Respiratory Diseases
- Ischaemic Heart Disease
- Hypertension
Promote and co-ordinate womens health
To allow all women an option to see a female practitioner for assessment should they wish. To include:
- Pre-conceptional care
- Contraception
- Cervical cytology
- Menorrhagia
- Management of the menopause
- Hormone replacement therapy
- To promote breast awareness to the practice population
Pathological specimens and investigatory procedures
- Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures.
Administration and professional responsibilities
- Participate in the administrative and professional responsibilities of the Practice team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
- Attend and participate in Practice meetings as required
- Support the practice in achieving Quality and Outcomes Framework targets
Training and personal development
- Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
- If it is necessary to expand the role to include additional responsibilities, full training will be given.
- Develop and maintain a Personal Learning Plan
Liaison
- As well as the nursing team there is a need to work closely with reception and administration staff to ensure the smooth running of the Practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team.
- There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care
Continuing Education
- Continue to update through relevant literature, especially in relation to NSFs and NICE recommendations.
- Maintain awareness of clinical governance issues.
- Contribute to practice based guidelines, protocols and the practice development plan.
- Annual appraisal and to undertake development identified here.
