Salary: £43,017.00 £46,928.00per annum pro-rata (depending on experience)
We are looking to welcome an Advanced Nurse Practitioner to join our On-The-Day-Team; our On-The-Day-Team currently consists of GPs, Nurse Practitioners, Paramedics and a Clinical Pharmacist. We are a training group, and offer development for our staff including external and internal training opportunities.
Main duties of the job
The successful candidate will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills.

Job responsibilities
Clinical
- Work autonomously and be accountable for own professional actions
- Undertake telephone triage work, telephone calls and face to face appointments
- Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to all patients accessing primary care services. In addition, to make any necessary referrals to other primary or secondary care teams
- Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions.
- Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow up.
- Prescribe medication and treatment in accordance with NMC guidelines for non-clinical prescribers and NHS North Devon/Cornwall formulary
- Report, discuss and refer areas on clinical uncertainty with on-call GP, secondary care or partner organisations
- Where appropriate, review and action patient letters/discharges/results
Administration and Professional
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Group
- Attend and participate in Group meetings as required
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to practice
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
- Utilise Interpreting Support Services where appropriate
Delivering a quality service
- Recognise and work within own competence and professional code of conduct as regulated by the NMC
- Deliver care according to Group policies, local guidelines, SIGN, NICE guidelines and evidence-based care
- Consider and aim to deliver most cost effective prescribing, working towards Group and health board prescribing targets
Training and personal development
- Participate in the education and training of students of all disciplines and the introduction of all members of the Group staff where appropriate
- Develop and maintain an in house nurse-training programme reporting to the Nurse Practitioner Lead.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring requirements are met.
- Develop, maintain and be responsible for own Personal Learning/Development Plan
- Partake in regular supervision for own development
- Assist in the mentorship and assessment of training nurse practitioners and OTDT members
- Act as a mentor for pre-registration students and other staff, if appropriate

- Ensure good recordkeeping and maintain confidentiality in accordance with NMC guidelines
